Welcome to the Online Team Entry form for all junior teams within Maitland Basketball Association.

This form is for TEAM NOMINATIONS.

INDIVIDUAL PLAYERS - PLEASE DO NOT COMPLETE THIS FORM

PLAYERS LOOKING FOR A TEAM - Please refer to our website to complete an EOI

Team Nomination is subject to the following:
- nomination of minimum 7 players
- Payment Per player fees must be paid before taking the court in any games
- Grading process as per our competition by-laws
- All players must maintain a current membership (BNSW/MBA) for the duration for the competition.
- EACH TEAM IS REQUIRED TO HAVE 2 TEAM CONTACTS, BOTH OVER 18 YEARS OF AGE.
