You should complete this form only if your team captain has completed the team registration. If you haven't heard anything from them, double check before beginning.
There are a couple of other things that you should be aware of before starting:
1. You will need to pay in order to complete the registration - the system is based on card payment. Contact the tournament director if you need an alternative.
2. You will need to upload your highest level of WFDF rules accreditation to your member profile before the tournament. This process only needs to be completed once as long as the accreditation is valid.
