For the "Hockey4All Disability Information", please select OTHER if you don't have any of the Disabilities
When filling in for a family, the first questions will apply for the Adult (If applying for an Adult), then after selecting the relevant Membership Fee per person you are registering, and continuing, you will then go through each child you are registering. Only after confirming all of the selections, you will proceed to the payment page.
DO NOT select all of the membership fees in the first register page, only select the membership fee for each user you are registering.
